Outpatient Clinic and Second Chances Program


At West Oaks Outpatient Clinic we provide a full continuum of outpatient mental health services for adults, adolescents, and seniors. Our entire staff has been trained in a social learning and positive reinforcement model which focuses on encouraging positive behaviors, attitudes, skills, and strengths, while teaching the life and coping skills needed to live a full, enriched life.

Our staff consists of licensed clinical therapists, nurses, psychiatric technicians, and financial counselors. Initial assessments are always free and the program offers free transportation, breakfast, lunch and snacks.

Our Individual Programs Offer

Adult, Senior or Adolescent Partial Hospital Programs Monday thru Friday 9:00am – 2:30pm:

  • Daily nursing for medication management, illness education, and support.
  • Medication management and treatment by a board certified psychiatrist at least two times a week
  • Daily Group Psychotherapy with licensed psychotherapists
  • Weekly Family Group Psychotherapy
  • Individual and Family therapy as needed with licensed psychotherapists
  • Lunch and snacks

Adult, Senior, or Adolescent Intensive Outpatient Programs Monday thru Friday 9:00am - 12:00pm

  • Daily Group Psychotherapy with a licensed psychotherapist
  • Family Therapy as needed with a licensed psychotherapist
  • Weekly Family Group Psychotherapy
  • Lunch and snacks

Chemical Dependency Partial Hospitalization and Intensive Outpatient Programs for Adolescent, Adults, and Seniors:

Substance Abuse Treatment Monday to Friday

  • 9:00 am – 2:00 – Partial Program (Adults/Seniors)
  • 9:00am – 12:00pm – Intensive Outpatient Day Program (Adults/Seniors)
  • 6:00pm – 7:30 pm – Intensive Outpatient Evening Program (Adults)
  • 9:00am – 12:00pm –Intensive Outpatient Program (Adolescents)

All of our Substance Abuse Programs Offer Daily Group Counseling for:

  • Relapse Prevention
  • Life Skills Training
  • Substance Abuse Education 12 Step Concepts
  • Mental Health Education
  • HIV and STD Education
  • Multifamily group weekly
  • Individual Counseling
  • Assistance with referrals
  • Lunch and Snacks
  • Free after care for up to one year

Our Partial Hospitalization Program also offers;

  • Daily nursing for medication management, illness education and support
  • Medication management and treatment by board certified psychiatrist at least two times a week

Second Chances Partial Hospital and Intensive Outpatient Program

Monday thru Friday: 9:00am – 2:30pm

Based on the successful New York Presbyterian/Cornell Second Chance Inpatient Program, West Oaks’ Second Chances Program focuses on working with chronically mentally ill patients who have had multiple inpatient hospitalizations, and have shown little progress with other outpatient treatment.

Using a social learning model, positive reinforcement, token economy, and level systems, this evidenced based program focuses on:

  • Building practical life and social skills
  • Medication compliance
  • Reducing or eliminating inpatient hospitalizations
  • Increasing the level of independent living

Collaboration with patients’ family or other support systems is key in this treatment program:

  • Free training is offered to all caregivers.
  • Weekly meetings and frequent communication is held with all persons involved i.e. case managers, group home owners, family members, and legal guardians.

Patients will be assessed individually for appropriateness for this program.
